The Hospice and Palliative Care Nurse Practitioner contributes to the Hospice and Palliative Care program by providing consultation to patients and families faced with chronic or life limiting illness. Provides expert pain and sympton management, promotes communication with the health care team and facilitates discussion related to end of life diagnosis and prognosis. Promotes the concepts of palliative care to improve quality of life through collaboration with physicians and interdisciplinary team members involved in referred patients' care. Two PRN positions available. Flexible hours/days.

Qualifications: Must be a licensed Advanced Practice Nurse in the the State of Ohio. Currently licensed to prescribe in the State of Ohio. Master's Degree required. Strong communication, interpersonal and problem-solving skills required. Previous Home Care and Hospice experience preferred.
