Manager, Patient Services - Home Care

Under the direct supervision of the Director, VNS/PCS, the Manager, Patient Services is responsible for day-to-day oversight of patient care services delivered in an assigned area. Facilitates delivery of effective, efficient, and quality care. Works collaboratively with members of the team to help meet positive patient outcomes.Oversees the day-to-day operations of a Care team. Monitors productivity, referral trends, and quality outcomes. Performs supervisory visits to ensure performance, competency, and accuracy with documentation, and overall job expectations. Manages scheduling for staff to ensure adequate coverage on a daily basis, including weekends and holidays. Evaluates appropriateness of referrals for homecare. Maintains knowledge base of services and criteria for accepting referrals Facilitates team meetings to facilitate education and updates to staff. Evaluates ongoing processes to achieve operational and financial excellence. Ensures OASIS data, physician orders, and other required paperwork is complete, timely, and accurate Maintains knowledge and adheres to regulatory and legal requirements of Medicare, Medicaid, and third party payer sources. Ensures that services provided meet standards for reimbursement, when appropriate Oversees staff in providing quality care, utilizing resources that are appropriate, safe, and cost effective Performs annual and interim performance evaluations. Uses corrective action on an appropriate and timely basis to address employee performance concerns. Maintains competencies specific to patient population served. Maintains RN licensure and CPR certification.

Qualifications: Bachelor Degree in Nursing or related field or five years of experience in supervisory role in home health care. Currently licensed in the State of Ohio as a registered nurse. Strong communication and interpersonal skills. Ability to prioritize multiple tasks in a fast paced work environment.
