Under the direction of the Manager Patient Services, the Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient outcomes.
•Implements plan of care and perform nursing procedures, which have been assigned by the registered nurse.
•Provides direct patient care as defined in the state Nurse Practice Act.
•Assesses and provides patient and family/caregiver education and information pertinent of diagnosis and plan of care.
•Observes and assesses the medical, physical, socioeconomic, and cultural interactions and changes in the patient environment.
•Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse.
•Assumes responsibility for meeting productivity expectations and the staffing needs of the agency. Demonstrates flexibility when staffing needs require schedule changes.
•Communicates information to care team/supervisory staff and collaborates with disciplines, physicians, agencies and payer sources to facilitate excellent patient care. May participate in nursing and interdisciplinary care conferences.
•Performs accurate, concise daily documentation of observations, assessments, and interventions and the proper disposition of written records. Updates patient records in accordance with agency guidelines.
•Maintains competencies specific to patient population served.
•Completes mandatory education and training in order to maintain organization and department-specific competencies and requirements. Maintains applicable certification/licensure.
•Follows hospital/system and department policies and procedures with special attention to attendance and punctuality, confidentiality, dress code, and display of ID badge, and safety.
Qualifications: Currently licensed in State of Ohio as a Licensed Practical Nurse. One year of community/home health or medical/surgical experience preferred. Strong communication and interpersonal skills. Ability to prioritize multiple tasks in a fast-paced work environment.
