Edwin Shaw Rehab is currently seeking an RN to work 2nd shift, 3:00 p.m. - 11:30 p.m.
Under the direction of the Director, Nursing/Patient Services-ESR, the RN is responsible for providing professional patient care at a competent level of nursing practice according to policies/procedures, clinical knowledge and experience, nursing standards and patient needs.
Prioritizes patient assessment based patient needs, existing signs/symptoms/data, age related and developmental changes according to policy. Recognizes and addresses potential problems, abnormal findings and changes to patient's condition. Initiates actions to eliminate or minimize risk. Completes ongoing assessment according to policy.
Develops individualized plan of care for patients based on desired outcomes and assessment of patient's needs (e.g. physical rehabilitation, psychosocial, patient's perceptions, age-related/population and developmental, teaching for patient/family, and discharge planning).
Revises and prioritizes nursing care activities for assigned patients based on patient's response and overall effectiveness of plan (e.g. patient needs/age related issues, patient activities off unit, patient/family preferences, emergencies, changes in staffing and other available resources).
Provides direct patient care that is efficient, organized, timely, cost effective and considers patient satisfaction. Independently performs clinical procedures to achieve expected outcomes by demonstrating rehab competency skills, adhering to policies/procedures and ensures patient comfort, safety and privacy.
Documents assessment data, nursing interventions and patient care outcomes according to nursing standards, policies and procedures. Obtains and reports pertinent patient care information to appropriate person.
Collaborates with physicians and other health care team members to coordinate medical and nursing management of patient care.
Serves as a patient/family advocate to facilitate patient care, communication and resolution of problems. Represents patient's needs to others. Identifies barriers of communication and initiates appropriate action.
Develops and documents individualized teaching plan by assessing patient/family learning and referral needs. Implements plan to meet needs prior to discharge or transfer.
Identifies and collaborates with unit manager to implement planned improvement. Participates in projects by gathering data, providing information, identifies trends and implements findings of research/quality studies as directed.
Provides leadership/organizational skills by delegating appropriate care to assigned staff based on staff educational and competency level. Participates in orientation of new staff and serves as a resource for students. Serves on committees and participates in activities as required.
Completes mandatory inservice educational programs and competency testing by required renewal dates. Establishes learning goals and actively pursues required knowledge and skills to stay up to date on technical knowledge, new equipment and procedures.
Demonstrates professional behavior, quality care and concern for patients and guests.
Completes mandatory education and training in order to maintain organization and department specific competencies and requirements. Maintains applicable certification/licensure.
Qualifications: Graduate of an accredited school of nursing. Current lincense as a registered nurse in the State of Ohio. One and one-half years clinical experience in rehabilitation or two years medical-surgical experience preferred. CRRN preferred. Mastery of clinical nurse competencies within probationary period required. Must maintain clinical nurse competencies by satisfactorily completing/passing all mandated programs (including CPR certification) as required by policy and population served. Ability to work alternate or partial shifts. Effective interpersonal skills that are indicative of a customer service philosophy caring standards and continuous improvement.
