Care Management Care Managers are available to assist you and your family with your care and help you plan for your discharge. They can answer questions about community services and assist in making arrangements for follow-up health services, such as home healthcare, nursing home care and home medical equipment. Insurance companies work closely with these staff members to understand the care and services you will need at discharge. Clinical Social Workers are also available to assist you and your family.
Advance Care Planning Your admission packet contains a pamphlet, "You Have the Right: Using Advance Directives to State Your Wishes About Your Medical Care." It explains your options for directing your healthcare. If you did not receive a copy, please ask your care provider for one.
Healthcare Providers Outside of Edwin Shaw Rehab During your stay, you may have an appointment scheduled outside of the facility. This appointment could be a follow-up visit with your surgeon, a visit with a specialist, dialysis, radiation treatment or any other scheduled appointment. Your doctor will order the mode of transportation you require.
This notice is to inform you that Medicare considers ambulette (wheelchair van) transports a non-covered service. Private insurance or insurance through an employer may or may not cover this service depending upon your policy. Please discuss any concerns or questions with your doctor or the care manager on your nursing floor.
