Care Managers are available to assist you and your family with your care and help you plan for your discharge. They can answer questions about community services and assist in making arrangements for follow-up health services, such as home healthcare, nursing home care and home medical equipment. Insurance companies work closely with these staff members to understand the care and services you will need at discharge. Clinical Social Workers are also available to assist you and your family.
Advance Care Planning
Your admission packet contains a pamphlet, "You Have the Right: Using Advance Directives to State Your Wishes About Your Medical Care." It explains your options for directing your healthcare. If you did not receive a copy, please ask your care provider for one.
