When your doctor decides you are ready to leave Edwin Shaw Rehabilitation Institute, he or she will write an order for your discharge. Please have your driver park by the main entrance. A staff member will escort you to the entrance and remain with you until your transportation arrives.
Before You Leave
We will give you information about your medicines, when you can go back to work, school or other activities. If you are unsure of what you will need to go home, you can ask about:
- A complete list of medicines you should be taking (how and when you will get them)
- Foods you should and should not have
- Special needs (such as having assistance at home) or equipment
- Follow-up doctors' appointments or tests
- Symptoms that will alert you to call your doctor
- Activities you can and cannot do
- Flu and pneumonia vaccines

Medical Equipment & Supplies
Visiting Nurse Service & Affiliates offers medical equipment and supplies to patients and caregivers. For more information, ask your Care Manager or call 330-434-1114.
Billing
During your stay, your care manager can answer questions about insurance coverage, Medicare, or payment arrangements. We do encourage you to contact your insurance carrier with concerns or questions about your insurance. We will make every reasonable effort to contact your insurance carrier so you will receive all the benefits to which you are entitled. We will also bill your insurance company and research the possibility of another source of payment, particularly before Medicare is billed.
