Your Care
You and your family will be seen by a personal physician who will coordinate all of your medical care. This physician is part of a health care team, which includes a nurse and other physicians. If urgent medical problems arise when your family physician is not available, you will most likely be seen by another physician on your healthcare team.

The associate resident physicians practicing at the Akron General Center for Family Medicine are receiving specialized training. We may occasionally ask patients to assist in their training by permitting us to videotape the physician's visit with them. By participating, patients can help resident physicians enhance their knowledge and skills to better serve.

Medical students from Northeastern Ohio Universities College of Medicine (NEOUCOM) also receive training here. They do not provide medical care, but at times you may be asked to allow a student to interview or examine you. This will always be done only with your permission and under the direct supervision of a physician who provides your actual care.

Notice of Privacy Practices

Referrals
For more specialized care, your family doctor may refer you to another physician. Most insurance companies require this referral from your doctor in order for their charges to be covered. In order to ensure insurance coverage, please discuss referrals to other physicians with your family doctor before making an appointment. If you see another physician outside of our office, please notify us so that we can keep your records current and provide appropriate follow-up care.

Each insurance company has different policies regarding referrals for specialist visits and ancillary services. Please review your insurance booklet so you will know the requirements of your insurance company. When your family physician refers you to a specialist or for an ancillary service, our referral office will notify your insurance company if a referral is required. A copy of the referral will also be faxed to the specialist.

If you call our office to request a referral, a message will be forwarded to your physician for review. If the referral is authorized, our office will follow your insurance company requirements and will also notify the specialist. If the referral is not authorized, you will be notified. Our office needs at least 48 business hours to process these requests. Our office cannot authorize or process retroactive referrals.

Prescription Refills
Patients may have their prescriptions filled at the Ambulatory Care Center pharmacy, located on the fourth floor. To request refills, call the office phone at 330-344-6047 (option 2), 24 hours a day. Have your prescription number and pharmacy phone number ready. Refills will be called into your pharmacy within 24 hours during weekdays. Patients are asked to call us with their refill requests during regular weekday office hours.

Care After Hours
A physician is on-call at all times in the event that a serious illness develops. Patients should call the office at 330-344-6047 (option 0) as soon as possible. After hours, an answering service will take your call and relay your message to the on-call physician, who will call to assist you with your problem. This on-call service is only for emergencies that can't wait until regular office hours.

Prescription refills, test results and other routine requests are handled only during office hours.

Emergency Care
Dial 9-1-1 in a life-threatening emergency. Otherwise, call us before going to the Emergency Department at Akron General Medical Center or Akron Children's Hospital. Many urgent care services can be provided right in our office, including stitches and treatment for broken bones, acute asthmatic conditions and other minor emergencies. If necessary, a physician may meet you at our office or in the emergency room. Most insurance companies require prior approval from your physician for reimbursement of emergency visits, ambulance and other emergency services.

If you have an emergency during regular office hours, call our Emergency Line at 330-344-6651. After hours, call 330-344-6047.

Our Office is Online
This new service allows you to request appointments, refill prescriptions, and communicate medical issues about your health directly to your Center for Family Medicine Center doctor. It is not to be used in case of emergencies or for your urgent care needs. Simply talk to our front desk staff to see if you are eligible to use this service. Insurance Information
The Akron General Center for Family Medicine participates in many insurance plans. To find out if we are listed with your plan, review your benefits package or call our billing office at 330-344-6047 (option 4).

Be sure to bring your insurance card with you to each visit. Our office staff will file your insurance claims if you provide us with the appropriate information. Insurance companies usually require a co-payment at the time of service.

Fees
We accept payment by cash, check, Visa or MasterCard. We offer payroll deduction for Akron General employees. Full payment or insurance co-payment is required at the time of your visit unless other arrangements have been made. The parent accompanying a minor or child is responsible for payment of care. Minors cannot be seen without a parent or a signed request by the parent for us to provide care with few exceptions.

If payment is not received from your insurance company after 60 days, you will be sent a statement requesting payment. After 90 days, delinquent accounts are subject to collection. A monthly payment plan is available by special request.
