Our Social Services team is dedicated to providing caring patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of functioning and enhance our patients' well-being.
Social Services provides assistance with discharge planning such as linking patients to available community resources, and identifying eligibility for services and benefits, nursing home placement information, alternate living arrangements and financial resource information to fit individual needs and eligibility.
Services can be requested by a physician, any member of the healthcare team, agencies serving the patient prior to or after admission and directly by the patient and family. There is no charge for these services.
