You have
the option of completing Advance Directives. Advance
Directives are decisions you make about life-sustaining
treatments. They include a Living Will and Durable
Power of Attorney for Health Care. A Living Will
allows you to indicate preference regarding withdrawal
of life-sustaining treatment if you are no longer
competent. Durable Power of Attorney for Health
Care allows you to appoint someone to make health
care decisions for you if you are not able. If you
have chosen to have Advance Directives, please have
your paperwork completed prior to your day of surgery.
The day of surgery is not the appropriate time to
complete an Advance Directives. Please complete
in advance and bring a copy with you to be included
in your medical record.
If you have questions about
Advance Directives or need further assistance, please
call 330-344-6880.
