Rachel Baldi, MD
Rachel Baldi, MD, is board certified in Internal Medicine. Dr. Baldi attended the Medical College of Ohio and completed her internship and residency at MetroHealth Medical Center in Cleveland. Prior to joining Lodi Community Care Center in 2005, Dr. Baldi was the medical director at Bridges Home Health Care and an associate of Internal Medicine Associates in Medina, OH.

Tonia Grabowski, CNP
Tonia Grabowski, CNP, is a Certified Nurse Practitioner. Tonia received her Bachelor in Nursing from Case Western Reserve University, where she also went on to pursue her Masters in Nursing and become a Certified Gerontologic Nurse Practitioner. Tonia joined Lodi Community Care Center in June 2009. |

Kimberly Sheets, DO, is board certified in Family Medicine. Dr. Sheets attended the Ohio University College of Osteopathic Medicine and completed her internship and residency at Barberton Citizens Hospital. Dr. Sheets joined Lodi Community Care Center in July 2004. |
