Your Guide to Surgery at Akron General Medical Center


Please Bring with You

Please bring the following with you on the day of your pre-surgical testing or on the day of your surgery if you did not have pre-surgical testing:

  • Health care insurance card.
  • This booklet and a list of any special equipment (i.e. insulin pump, oxygen tank, c-pap machine, etc.) that is used at home.
  • A copy of your completed Advance Directives. There may not be sufficient time available the day of surgery to complete. (optional - see pg. 12 for description).
  • Your patient health history completed in black ink (form provided in the front of this booklet) and a complete listing of medicines with dosages that are prescribed or over-the-counter medications.
  • Any doctor orders.
  • Any completed tests from another facility or doctor (i.e. test results, EKGs, X-rays).
  • Your completed History and Physical, if completed in the last 30 days by your primary care doctor or any other doctor. (Otherwise, a History and Physical may be completed and billed to you or your insurance company.)
  • Your parking ticket.
