Clinical Social Worker Inpatient Psychiatry LISW/LSW required
Position Summary:
Under the direct supervision of the Director, Clinical Social Work, the Clinical Social Worker provides psychosocial assessment, counseling, crisis intervention, and community resource assistance on a consultation basis to patients and families utilizing a variety of treatment modalities, including individual, family, group and specialized counseling; works collaboratively with the multidisciplinary team to effect positive patient and family outcomes.
Age Specific Criteria:
Newborn: Educates parents on how to develop communication and emotional bonds with infant, how to recognize normal responses to meet infant`s environmental and physical needs, and how to provide social stimulation.
Newborn: Educates parents on resources to meet and maintain normal growth and development (e.g., immunization shcedules), to meet safety needs (e.g., car seats, shaken baby syndrome),and to obtain support for special needs (e.g., PEP, support groups, parenting classes).
Newborn: Evaluates for child abuse or neglect issues.
Adolescent: Educates adolescents about risk behaviors which may result in unplanned pregnancies, which may compromise sexual health (e.g., STD`s), or which may risk physical well-being, or result in traumatic injuries through under-age drug or alcohol use.
Adolescent: Intervenes and educates adolescents on adjustment to illness issues when a parent or another family member is ill and unable to resume or maintain usual parental role and interactions to help the adolescent cope with role changes and conflicts.
Adolescent: Ensures educational needs are met during periods of illness or hospitalization.
Adolescent: Evaluates for signs or symptoms of child neglect or abuse.
Geriatric: Promotes patient self-determination within the context of the patient`s physical and mental strengths and limitations to maintain an appropriate degree of independence; fosters and involves the patient in decision-making about life transition issues; and assist the patient in coping with adjustment to changes or losses related to chronic illness, mental status changes, income limitations, retirement, and relationship changes or losses.
Geriatric: Educates and involves appropriate family members and/or caregivers in supporting the social, educational, leisure, physical and mental needs of the geriatric patient.
Geriatric: Assists and provides supportive services and education to family members and/or caregivers in coping with and accepting the physical, mental, and relationship changes which may occur between spouse or adult child and an aging patient who may be experiencing acute or chronic physical or mental health issues.
Geriatric: Evaluates for signs and symptoms of elder abuse, neglect, or exploitation including self-neglect.
Qualifications: Job Duties:
Conducts psychosocial assessment interviews with patients and families to identify problems requiring social work interventions and treatment plans for appropriate services.
Provides counseling to impact problems related to environmental needs, family or interpersonal conflict, grief issues, financial concerns, and coping with physical and/or mental illness using a variety of treatment modalities, including individual, family, group or specialized counseling.
Intervenes in crisis situations.
Assists patients and their families in adjusting to episodes of acute and chronic illness; coping with lifestyle changes precipitated by illness or injury and addresses issues of terminality.
Provides consultation to Care Managers and other health care professionals when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes; receives referrals for complex problem resolution from Care Managers, physicians and/or care team.
Identifies community resources and provides information and referrals to same.
Updates and maintains professional knowledge, resource information, skills and competencies related to areas of practice and assigned caseload duties by participation in department staff development and other professional education programs and by development of annual professional goals and activities to meet these goals.
Documents social work assessments, interventions, and psychosocial treatment plans according to department and medical center guidelines; compiles statistical reports, prepares specialty documents and correspondence as required.
Participates in health system and departmental Performance Improvement Initiatives to effect positive patient and family outcomes.
Adheres to NASW Code of Ethics.
Completes mandatory education and training in order to maintain organization and department specific competencies and requirements. Maintains applicable certification/licensure.Minimum Qualifications:
Master's degree in social work with a casework concentration from a CSWE accredited program.
Ohio license appropriate to assigned caseload and practice area.
One to two years direct practice experience.
Strong interview assessment organizational and problem solving skills.
Ability to work collaboratively with patients families multidisciplinary team and community agencies.
Strong interpersonal skills.
