Hospice & Palliative Care APN

Position Summary: Under the supervision of the Vice-President, Nursing and a standard care arrangement with an identified Akron General physician, the Clinical Nurse Specialist Hospice & Palliative Care collaborates with medical staff and other clinical disciplines for those patients who have consults for palliative care. The CNS evaluates, plans, and directs interdisciplinary care to meet the palliative needs for these patients. Job Duties: Performs focused beside evaluation and assessment of patients. Assists patient/family in identifying and articulating goals of care. Assists patient/family in establishing preferences for heroic measures/DNR status. Provides recommendations for health maintenance, health promotion, palliative care, and end of life care, including alternate care settings. Assists with diagnosis, treatment, and management of acute and chronic health conditions. Consults Palliative Care physician as needed. Provides orders that reflect appropriate lab and testing procedures to facilitate proper treatment and management. Orders treatments and durable medical equipment and performs therapeutic or corrective measures as indicated. Provides sufficient information to the patient and family for informed decision-making, as appropriate. Communicates decisions and care needs to the interdisciplinary team, including the palliative care physician as well as the referring/attending physician. Develops, coordinates, and communicates a care path for the patient. Documents and maintains accurate patient records, charts and documents to support scope of practice and potential reimbursement for services provided. Provides education and training for staff, medical residents, and students. Completes mandatory education and training in order to maintain organization and department specific competencies and requirements. Maintains applicable certification/licensure. Follows hospital and department policies and procedures with special attention to attendance and punctuality, confidentiality, dress code and display of ID badge, and safety. Qualifications: Minimum Qualifications: Master's degree in Nursing from an accredited CNS program. Licensed as a Clinical Nurse Specialist in the State of Ohio. National Certification from the American Nurses' Credentialing Center. Certification in palliative care preferred. Competency in advanced care planning.
